How the body breaks down HA
The HA in filler is cross-linked to resist the body's natural hyaluronidase enzyme. The tighter the cross-linking, the longer it lasts. Breakdown is continuous from day one, but slow at first and faster once the gel structure starts to fragment. Most men therefore notice a stable size for around 8–12 months, then a gradual decline over the next 6 months.
Factor 1: gel type
Body filler is several times more viscous and more cross-linked than facial filler, and designed to withstand pressure and movement. If you were injected with a soft facial filler, the result usually lasts only 4–8 months. This is the number one cause of early fading that the doctor sees.
Factor 2: injection layer and distribution
Gel that sits in the correct layer beneath the skin and spreads as an even sheet has less surface area in contact with tissue than gel scattered in small deposits, so it breaks down more slowly. Injecting too shallow not only feels beady but also fades faster.
Factor 3: volume
Too little volume, such as 3–5 cc spread along the whole shaft, seems to disappear quickly because losing just 30% is enough to make the result invisible. An adequate volume from the first session makes the result feel like it lasts longer, even though the rate of breakdown is the same.
Factor 4: use and habits
Sex or masturbation in the first 14 days, before the gel has settled, shifts the gel and spreads it into the wrong places. After that, normal use does not speed up breakdown in any meaningful way. Heavy exercise, saunas and regular massage of the area increase the rate of breakdown slightly.
Factor 5: your own body
Metabolic rate, natural enzyme levels and tissue response differ from man to man. Some men break HA down faster than average for no correctable reason. If the first session fades early even though everything was done correctly, the doctor usually changes the gel type or moves to more frequent top-ups with smaller volumes.
Signs it is time to top up, and how to top up economically
When the measured circumference has dropped around 30–40% from its peak, or it no longer feels as full as before. Topping up while some of the original gel remains takes less volume and gives a smoother result than letting it disappear and starting again. The doctor checks with ultrasound where the existing gel sits before every top-up.
