Olive oil, Vaseline and paraffin injections

Olive oil or paraffin injection removal: how a paraffinoma is repaired

Men have been injecting olive oil, Vaseline or paraffin for size for a long time, and we still see the results. The body cannot break these substances down. Sooner or later they show up as hard lumps, darkened skin or pain. The fix is surgery. The doctor examines you, plans it and refers you to the specialist team.

What happens when oil is injected

Olive oil, coconut oil, baby oil, Vaseline and paraffin are substances the body cannot digest. It tries to wall them off with inflammation and fibrous tissue, which becomes the hard lump doctors call a paraffinoma or sclerosing lipogranuloma. Some men get it soon after the injection; others only years later.

What are the symptoms?

  • Hard nodules you can feel, or rough, rippled skin
  • Tight, discoloured or darkened skin
  • Swelling or a distorted shape
  • Pain during erection or intercourse
  • Foreskin that cannot be pulled back
  • In some cases, skin thins until it ulcerates or breaks open
Why it cannot be dissolved or sucked out

The enzyme hyaluronidase dissolves HA only. Oil and paraffin sit in the tissue and fibrous scar as tiny droplets. They cannot be suctioned or squeezed out, and trying only makes the inflammation worse.

How is it treated?

Ultrasound assessmentShows the extent of the substance and fibrous tissue. Some men have an MRI as well to plan the surgery.
Planning and referral for surgeryThe doctor assesses with the specialist team how much tissue needs to come out and how the skin will be repaired, then refers you to a suitable operating theatre.
Surgical removal of affected tissueAs much of the lump and fibrous tissue as can be removed without harming healthy tissue.
Skin repairUsing nearby skin, such as scrotal skin, or a skin graft. Some men need surgery in 2 stages.
Wound care and follow-upAppointments until the wound has fully healed, then a talk about whether any further refinement is needed.

Can I have filler after the repair?

In some cases, yes. The wound has to heal fully and the doctor has to confirm the tissue is ready. We do not inject filler over an area where oil or paraffin still remains, because it raises the risk of inflammation and makes repair harder.

Surgery to remove paraffin or oil: how it is done, and what happens afterwards

This section is for men who have decided to have it fixed, or are weighing whether surgery is worth it. It explains the methods the surgical team uses, how to prepare, and what you can realistically expect afterwards.

Two main surgical approaches

The first removes the oil-infiltrated tissue and closes the wound with your own remaining skin. This works when the oil is confined to a limited area and the rest of the skin is healthy. The second is for oil that has spread around the whole shaft, with thinned or ulcerated skin. The affected skin is removed entirely and covered with a skin graft taken from the thigh or scrotum.

The surgeon chooses the method from the ultrasound images and the physical examination, not from the patient's preference alone, because incomplete removal lets the lumps come back and makes a second operation harder than the first.

Why do suction, dissolving injections and laser not work?

Oil and paraffin do not sit in a pocket that can be sucked out. They spread as tiny droplets through the tissue, each wrapped in fibrous scar. No enzyme or injectable medication dissolves them. Laser reaches only the superficial skin. Attempting suction removes very little and leaves cavities that inflame easily.

If a clinic offers to dissolve oil with an injection, ask what substance they use and ask to see its registration documents.

Preparing for surgery

An ultrasound to map the extent, basic blood tests, and any inflammation or infection settled first. Stop smoking at least 2–4 weeks beforehand, because it affects how well a skin graft takes. Tell us about your regular medication, especially blood thinners.

Dr. Kate assesses and prepares you. The surgery itself is performed by a surgical team at a facility with an operating theatre and an anaesthetist. The doctor tells you the name of the facility and the operating surgeon on planning day.

Recovery and realistic results

Excision with direct closure takes about 2–3 weeks to recover. A skin graft needs dressings and limited movement for roughly the first 1–2 weeks. The wound heals fully in 4–6 weeks, and the grafted skin softens over several months. No sex until the surgeon clears you, usually 6–8 weeks.

The aim of surgery is to remove the substance and restore pain-free normal function, not to make it look perfect. Grafted skin differs in colour and texture from the original, and there will be scarring. The doctor shows you real results from similar cases in the examination room.

Frequently Asked Questions

I had oil injected many years ago. Can it still be repaired?

Yes, but the longer it has been there, the more fibrous tissue there usually is. The doctor scans the extent with ultrasound first, then tells you how much improvement is realistic.

Where is the surgery done? Do I need to stay in hospital?

It depends on the size of the operation. Small cases can be done under local anaesthetic; larger ones need a hospital operating theatre. The doctor tells you after the examination and helps coordinate the referral.

Will everything function normally after surgery?

The aim is to remove the problem substance, reduce pain and improve function. How much comes back depends on how severe each case is. The doctor tells you straight before you decide.

Will surgery affect my erections?

Oil sits in the layer beneath the skin, not in the erectile tissue. Surgery in the correct layer does not affect erections. Early on, erections will be painful and tight, and this improves as the wound heals.

How many nights in hospital?

Excision with direct closure usually means going home the same day or staying 1 night. A skin graft usually means 1–3 nights so the graft can be monitored. The surgeon decides.

Roughly how much does the surgery cost?

It depends on the method, the extent and the facility. The doctor can give you a price range after the ultrasound, and the surgical team confirms the final price before the operation is booked.

Can I have filler for size after surgery?

In some cases, once the wound has healed fully and the fibrous tissue has softened, usually after at least 6 months. The doctor checks on ultrasound that enough of the layer beneath the skin remains for gel to spread evenly. Some men who have had a skin graft are not suitable for filler.
